How much does Wegovy cost?

As of August 2026, Novo Nordisk publishes a Wegovy self-pay price between $149 and $399 a month depending on which Wegovy you mean and what dose you are on. Patients with commercial insurance that covers Wegovy can pay as little as $25 a month with the savings offer, capped at $100 in savings per month. Eligible Medicare Part D beneficiaries pay $50 a month through a federal demonstration that runs through the end of 2027. The catch behind that range is that there are now two Wegovys.

There are two Wegovys, and they are priced differently

The FDA approved the Wegovy pill — once-daily oral semaglutide 25 mg — on December 22, 2025, and Novo Nordisk launched it in the United States in early January 2026⁸. Drugs@FDA lists it under NDA 218316 as four tablet strengths, 1.5 mg, 4 mg, 9 mg and 25 mg, which is the titration ladder you climb to reach the maintenance dose⁹. The original Wegovy is still the once-weekly injection, sold as a pen. A search for "Wegovy cost" in 2026 returns prices for both without always saying which is which, so the first question to settle is the form, not the number. Wegovy self-pay prices, dose by dose

Novo Nordisk's own price guide, dated April 2026, lists the self-pay prices below. This pricing is available for local pharmacy pickup with a savings offer or for home delivery through NovoCare Pharmacy¹.

For the **Wegovy pill**, a month is one bottle of 30 tablets: $149 a month at 1.5 mg, $199 a month at 4 mg, $299 a month at 9 mg, and $299 a month at the 25 mg maintenance dose. A limited-time offer holds 4 mg at $149 a month, and Novo Nordisk's footnote dates that offer to August 31, 2026, after which 4 mg goes to $199¹.

For the **Wegovy pen**, a month is one box of four pens: $349 a month at 0.25 mg, 0.5 mg, 1 mg, 1.7 mg and the 2.4 mg maintenance dose. Patients new to the Wegovy savings offer or to NovoCare Pharmacy pay $199 a month for the first two monthly fills at 0.25 mg or 0.5 mg, an offer Novo Nordisk dates to December 31, 2026². The higher-dose Wegovy HD pen, 7.2 mg, is $399 a month¹.

Read those two paragraphs against each other and the pattern is clear: the pill is the cheaper self-pay route at every rung, and the gap at maintenance is $50 a month — $299 for 25 mg of tablets against $349 for 2.4 mg of injection. The intro rate runs the other way, since only the pen carries a two-month $199 step-in². Novo Nordisk reserves the right to modify or cancel these programs at any time, so treat every figure here as the price published in August 2026 rather than a rate you have locked.

With commercial insurance: $25, with a ceiling

If you have commercial, non-government insurance that covers Wegovy, the savings offer brings the copay to as little as $25 a month for any dose of the pill, the pen or the HD pen¹. The ceiling matters more than the floor: the offer is capped at a maximum savings of $100 a month, so $25 is what you pay when your plan's own copay is $125 or less³. A plan that puts Wegovy on a specialty tier with a $300 copay leaves you paying $200, not $25. The savings card is also a pharmacy-pickup product — Novo Nordisk's guide notes the copay pricing is only available for local pharmacy pickup with a savings offer, not for home delivery¹. Government beneficiaries are excluded from the $25 copay offer¹. They are not shut out of self-pay, though: NovoCare's savings page states that government-insured patients may use the program if they self pay and process outside of their insurance³.

Medicare: $50 a month, through 2027

Medicare's position on Wegovy changed on July 1, 2026. CMS launched a short-term demonstration called the Medicare GLP-1 Bridge that gives eligible Part D beneficiaries a $50 copay on certain GLP-1 drugs between July 1, 2026 and December 31, 2027⁵. Medicare's own fact sheet names the covered drugs — Foundayo tablets, Wegovy injection or tablet, and the Zepbound KwikPen only — and states the cost is $50 per month regardless of income⁶. Wegovy.com confirms all doses of both the pen and the pill are covered at that price⁴.

Two conditions on that $50 are easy to miss. The Bridge runs outside the Part D benefit, so the $50 does not count toward your deductible and no part of it counts toward your true out-of-pocket total; there is also no low-income subsidy applied⁵. And eligibility turns on not already having coverage: you qualify only if you are not eligible to receive a GLP-1 through your Medicare drug plan, and you are excluded if you have type 2 diabetes, moderate-to-severe sleep apnea, or fatty liver disease, since those diagnoses may already open a covered path⁶. Medicare also sets clinical criteria — age 18 or older plus a BMI of 35 or higher, or 30 with certain heart failure, hard-to-control high blood pressure or stage 3a-plus chronic kidney disease, or 27 with prediabetes or a prior heart attack, stroke or blocked arteries⁶. CMS has not published what happens after December 31, 2027.

The list price, and what changes on January 1, 2027

List price is the number a patient with coverage rarely pays out of pocket, and it is the number that drives what you pay if your plan uses coinsurance or you are still in a deductible. CMS puts a figure on it: for the selected-drug group it calls "Ozempic; Rybelsus; Wegovy," the CY2024 wholesale acquisition cost for a 30-day supply was $959¹⁰. That is a group figure, not a Wegovy-only one, and Novo Nordisk does not publish a current Wegovy list price on its own.

Two changes land on the same day. Novo Nordisk announced on February 24, 2026 that effective January 1, 2027 it will cut the list price of Wegovy injection 2.4 mg and tablets 25 mg to $675 a month, a reduction of roughly 50 percent, and the company said plainly that this does not affect direct-to-patient self-pay prices⁷. Separately, Medicare's negotiated maximum fair price for the same semaglutide group takes effect January 1, 2027 at $274 for a 30-day supply, a 71 percent discount from the 2024 list price; the per-package price for a box of four 2.4 mg Wegovy pens is $385.63¹⁰. Neither figure is what a cash buyer pays at the counter.

What to ask before you commit

Ask for the price at your maintenance dose, not the starting dose, because the Wegovy pill climbs from $149 to $299 and the pen's $199 intro rate expires after two fills¹². Ask which pathway a quote belongs to — savings-card copay, self-pay, or Medicare Bridge — since the three are separate programs with different eligibility and only one of them is open to you at a time. Ask what "one month" means, since Novo Nordisk defines it as one box of four pens or one bottle of 30 tablets².
